​REGISTERING YOURSELF ON THE CAREGIVER PORTAL
 
STEP 1:  Go to https://my.adlware.com/CPUB
STEP 2:  Click on “New Registration” to the left of the LOGIN button

STEP 3:  Fill in the “Caregiver Information” which includes:
     A) Your caregiver ID#
     B) The account ID which is 10394
     C) Click whether you’d like to confirm with either your email or
          your phone number and enter that information
STEP 4:  Click “submit”
STEP 5:  On the next page you will see your information.  If anything listed is incorrect, please contact the office.      
STEP 6:  Towards the bottom of the page, please create and enter a password and confirm the password.  (Passwords must be at least 8 characters long)
STEP 7:  Scroll down and click on the boxes to agree to the “Terms of Service” and the “HIPAA Compliance Associate Agreement”
STEP 8:  Click “submit”
STEP 9:  You will then see a screen stating you have registered successfully and to “click here” to close the window.  Click there.
STEP 10:  You will then be redirected to the login screen.  Enter your caregiver ID# as your username, the password you’ve just created and the account ID which is 10394.
 
You will now be able to see your schedule, confirm your schedule, update your task codes (ADLs) as long as the shift has not been verified by the office and see your client’s information including name, ID#, address and care plan.
